Little is known about Ratthew's background. Based on dialogue with Ron from Accounting, it is known that Ratthew once entered Ron's office. According to Ron, he chewed through reports of an experiment that Ron was not supposed to see.[1] During his first interaction with the player, Ratthew explains that being small has its advantages, as he hears and sees things others do not. According to him, this makes being small a dangerous job. Trusting them enough, he reveals his name to be Ratthew and advises the player not to trust anyone in the hospital.
